Why Preparation Matters

Essential Documents (Bring All)
1. Pathology
Original pathology report (hospital-stamped)
Unstained slides (白片): 10-20 consecutive sections on positively charged slides — critical for IHC re-staining and molecular testing
Paraffin-embedded tissue block (蜡块): If your home hospital allows; otherwise 20+ unstained slides
All IHC stain results: ER/PR/HER2/Ki-67 (breast); PD-L1, TTF-1, p40 (lung); CD markers (lymphoma); etc.
Molecular results: FISH (HER2, MYC/BCL2/BCL6, ALK), NGS panel reports, PCR results (EGFR, KRAS, BRAF, BRCA)
2. Imaging
DICOM files on USB/CD: CT, MRI, PET-CT, mammography, ultrasound — original DICOM, not just PDF reports
Written reports for all imaging studies
Prior imaging for comparison: especially pre-treatment scans if assessing response
3. Treatment Records
Complete treatment summary: Every chemotherapy regimen (drug names, doses, cycles, dates), radiation records (dose, fields, fractions), surgical reports
Response assessments: CT/MRI reports after each treatment phase
Current medications list: Including supplements and TCM products
Adverse events: Any serious drug reactions, hospitalizations during treatment
4. General Medical
Recent blood work (CBC, liver/kidney function, tumor markers)
Allergy history
Vaccination records (especially COVID, hepatitis B)
Insurance information
Passport and visa
Letter of referral if available
Digital Organization
Scan all paper documents as PDF
Organize DICOM files by modality and date
Save everything to a USB drive (and cloud backup)
Create a one-page summary: diagnosis, stage, key molecular results, treatments to date, current status, specific questions for Shanghai doctor
